We have our own on-site physical, occupational, speech, respiratory therapists and subonone medication-assisted therapy.  Our staff is dedicated to each personalized patient’s expectations.  Dr. Serota is our Medical Director, Guidestar Eldercare is a specialty medical group that provides bedside care to neurocognitively impaired residents and our substance use services are Ray of Hope and DeCoach. We offer an environment which encourages independence, creativity, and personal growth. We accept adults of all ages and are Medicare and Medicaid certified.
