UNC Health Johnston offers Continuity of Care to Aging Adults

Through its home care, home hospice and inpatient hospice services, UNC Health Johnston is poised to deliver the care and assistance you or your family member may need at any stage of life.

Home Care

UNC Health Johnston provides medically necessary services to treat, rehabilitate or restore homebound adults to their optimal health in the setting where they feel most comfortable. Patients qualify if they are homebound and/or have a medical need that requires the skilled care of a clinician.

Who Needs Home Care?

Home health care helps people 18 and older who have trouble leaving home due to: 

  • Disability
  • Recovery from surgery, a medical condition, or an injury
  • Weakness
  • A doctor must certify that you or a family member needs home care. Call our office for guidance.

Health Care Services

A nurse will work with you or your loved one, along with family caregivers, to make a personalized care plan. It may include services such as:

  • Disease management
  • Hospice care for terminal illnesses
  • Intravenous (IV) therapy
  • Rehabilitation, including occupational therapy and physical therapy
  • Tube feedings and other specialty services
  • Urinary (Foley) catheter care
  • Wound care

Home Therapy

Our physical and occupational therapists provide medically based evaluations and treatment techniques designed to regain the function you lost whether it was from a hospitalization, rehabilitation facility or surgery.

Home Hospice

Our home hospice care can help you or a loved one spend the final weeks or months of life in the comfort of your home. Trust our compassionate team to ease symptoms and provide support, so your family can focus on spending time together.

Adults qualify for home hospice care if:

  • A doctor certifies they are expected to live six months or fewer
  • They are no longer seeking treatment to cure or slow the progression of an advanced illness

Hospice caregivers can visit wherever you or your relative lives in Johnston County, including a private home, nursing home, or assisted living facility.

Expect us to treat your family with dignity while providing:

  • Emotional support and grief counseling
  • Medical equipment to make tasks easier, safer, and more comfortable
  • Medication for symptom relief
  • Nutrition counseling
  • Nursing visits as often as needed as identified by the Plan of Care

Adults qualify for admission if they:

  • Have a life expectancy of six months or fewer
  • No longer seek treatment to cure their illness
  • Need the level of care requested

Insurance Coverage for Services
