At Grace Hospice, our mission is for each employee to treat every patient as their own treasured loved one, by providing care that affords a quality of life which outweighs quantity of days, and by supporting caregivers as their hearts find healing.

Our team provides a compassionate, patient-centered approach to medical care and support for people during the last months of their lives and to their families. We are partnered with the Visiting Physicians Association and can offer a full continuum of care, which includes a physician’s visit to a patients home, for those who want to stay in their home and they live out their life with a life limiting illness. The Visiting Physicians Association (VPA) has been in business since 1993. It is the nation’s leader in house call medicine and geriatric home health care. With VPA’s advanced medical technology, we are able to diagnose and treat patients in their home where we know they feel most comfortable.

• Personalized plans of care created to meet the individual needs and wishes of the patient.
• Our Volunteers are available 7 days a week, and our RN’s are on call 24 hours a day, to cut down on unnecessary hospitalizations.
• Our highly skilled volunteers are trained to relieve primary caregivers by helping them with household chores, sitting with patients, or running errands for them. Perhaps the most important task, however, is their ability to be “good listeners.” Hospice volunteers also provide administrative assistance to the hospice care team by working in the office.
• Our team is responsive to your needs and can be at your home to do an assessment within a couple hours of your call. The admission process has been streamlined to only take 30 minutes to an hour max.
• We can assist our patients in applying for Medicaid, Aid and Attendance, and with enrolling for their VA benefits
