Nursing and Wound Care

The Best Choice Home Care Adult Nursing professionals provide nursing care services at home for adults and seniors dealing with chronic illness, injury or disability. These services are provided by registered nurses and licensed practical nurses and are available 24 hours a day, 7 days a week. Our board-certified Wound Care professionals provide technically advanced, surgical and non-surgical outpatient care to heal wounds that resist conventional therapy. These include:

  • Neuropathic ulcers in patients with diabetes.
  • Pressure ulcers due to immobility.
  • Venous stasis leg ulcers associated with poor circulation.

Our Wound Care Certified nurses assess each patient in a holistic manner to determine all necessary interventions. Improved clinical outcomes are associated with companies that have an interdisciplinary wound team. Our team includes: Patient, Nursing, Physical Therapy, Occupational Therapy, Speech Therapy and a Wound Care Specialist. We work in tandem with doctors and nurse practitioners to identify the best treatment plan. Our success is driven by being patient focused.

Physical and Occupational Therapy

Many therapies can be provided at home to speed recovery from illness, injury or surgery, including physical therapy, occupational therapy and speech therapy. Home based therapy allows clients to develop independence with daily activities in a convenient and comfortable setting, allowing families to be closely involved in the rehabilitation process. Our Physical Therapists help individuals regain physical motion and strength after events such as:

Preventative Care

At Best Choice, Preventative Care falls under two main categories – Fall Prevention and Diabetic Education.  We’re proud to offer expertise in both areas, allowing for patient focused, effective preventative care.

Fall prevention is a major focus of all care team members.  Fall prevention assessment is completed upon admission and therapy evaluation. Our skilled practitioners will evaluate the home environment and make suggestions for safety. During the certification period, your nurse will continue to monitor safety in the home. Therapy will work to increase independence, safety, muscle strength and endurance.

Our number one goal is to prevent falls.

Our certified Diabetic Educator will assess each patient in a holistic manner to formulate a patient specific diabetic management plan. We will work with you to develop a drug therapy plan that includes blood pressure and dietary changes, for patients with diabetic nephropathy.
