AseraCare Hospice is one of the nation’s premiere providers of hospice care and palliative medicine services. Our palliative medicine services support those who are living with a serious illness and our family-centered approach to hospice helps ensure that the needs and wishes of both our patients and their family members are met.

Our Professionals help manage the entire spectrum of challenges and needs related to a progressive illness or terminal diagnosis.

It is our objective to help reduce the stress and pressure on our patients and their families, allowing them experience an enhanced quality of life and make the most of their remaining time together. This office serves Howard, Grant, Clinton, Tipton, Madison, Montgomery, Boone, Hamilton, Putnam, Hendricks, Marion, Hancock, Morgan, Johnson, Shelby and Monroe counties.

***Symptom Management***

Pain and symptom management are the hallmark of palliative medicine. The symptoms of progressive illness – pain, shortness of breath, nausea, anxiety – can be distressing. PRIME provides expert, specialized treatment to help provide relief. You’ll regain a sense of control over your symptoms and pain, which will help improve your physical comfort and mental well-being.

***Goals of Care Planning***

Most people do not think about what type of care they would want if they were in a situation in which they were unable to decide for themselves. With PRIME, we’ll help you understand the likely path of your disease and what to expect. When you’re comfortable, we’ll also help you set goals for your care plan and assist you in defining decisions about your care ahead of stressful situations that may arise. If the time comes for an important decision to be made, it will take the pressure off you to make a quick choice or the burden off your family and friends to make a decision for you.

***Medication Management***

PRIME provides medication management to help ensure that you’re on the right medications at the right time. It is important for your care team to know every medication that you’re on to help ensure that your medications are the most appropriate for your current condition.

***Transition Management***

Moving between care settings can be stressful, but PRIME helps make these transitions as smooth as possible. We’ll help maintain coordinate your care between settings and facilitate communication so that all your healthcare providers are kept in the loop about your current condition.

Hospice Services

Our hospice care is provided by a medically supervised interdisciplinary team of professionals and volunteers. The types of support services we provide vary from patient to patient. Our objective is to reduce the stress and pressure on the patient and their family to allow them to make the most of their time together.

***Nursing Care***

Nurses play an essential role in our hospice team.  They are thorough and compassionate in caring for our hospice patients. Because pain control is often central to hospice care, our nurses will make every effort to relieve pain and make the patient as comfortable as possible. Our nurses have also been trained in the latest holistic care techniques for both patients and their families. Each facility has a registered nurse on staff 24 hours a day, 7 days a week.

***Social Worker Counseling***

Social workers help our hospice patients and their families deal with symptoms, psychological stress, and ethical dilemmas such as those associated with withdrawing or withholding treatment. They often counsel families and patients to help them understand the emotions they may experience during this difficult time. Social workers may work with community agencies to assist the patient and family members. In addition, a social worker may facilitate communication between patient and family, attempting to ensure that everything essential is resolved between the patient, family members, and friends.

***Spiritual Support***

Spirituality is universal, but its expression is personal and unique to each individual. Some patients and their families desire the rituals and traditions of their religion, while others want to meet their spiritual end-of-life needs in their own individual way. Our spiritual coordinators help patients and their families address their emotional and spiritual needs at the end of life, in accordance with each person’s individual beliefs and preferences.

***Personal Care Assistance***

We understands our patients and their families may want to focus on each other, not on details and paperwork. We offer personal care assistance to give families the freedom from worry about care and safety issues.

***Physical, Occupational and Speech Therapy***

We have registered physical, occupational and speech therapists on staff to meet individual needs. These might include physical treatment to improve movement and strengthen daily living skills; speech therapy to improve swallowing; and, occupational therapy to provide pain relief or improve comfort. We believe that rehabilitative therapy for hospice patients can enhance their quality of life.

***Nutrition Counseling***

AseraCare provides nutrition counseling to assist patients with dietary health needs or concerns. Our licensed registered dieticians support patients who may be a nutritional risk because of their physical health status or diagnosis, medications or family status change. Dietary education is tailored to specific health conditions.

***Bereavement Counseling***

Bereavement counseling supports people with emotional and psychological stresses following the death of a loved one. We offer a broad range of transition services including outreach, counseling and referral services to family members. Hospice bereavement counselors support families as they begin their first year without their loved one, helping them through holidays, anniversaries or other special days with calls, visitors or inspirational articles and poems.

Our bereavement counseling programs are designed to:

  • Share feelings with others who have experienced or are experiencing loss
