PACE programs like Piedmont Health SeniorCare Services thrive with successful partnerships between participants, caregivers and provided services. Participants of Piedmont Health SeniorCare receive an individualized plan of care managed by their personal interdisciplinary team of health care professionals. Staff is on-call 24 hours a day, seven days a week, including holidays.

• Primary and specialty medical care, including cardiology, nephrology and orthopedic surgery
• Nursing care and nursing home care as needed
• Hospital, long term care, home health care and personal care
• Foot, vision, hearing, dental and psychiatric care
• Occupational, physical and speech rehabilitation therapies
• Prescription medications(including home delivery) and durable medical equipment(including wheelchairs and walkers)
• On-going suuportive case management and laboratory services and diagnostic testing
• Emergency medical and ambulance services
• Social work, preventative services and education(including nutritional counseling)
• Caregiver and family support, meals, transportation and recreational activities
